Lip Dermal Graft Recovery Abroad: A Week-by-Week Timeline

Short answer: After a lip dermal graft, most people need one to two weeks before they feel comfortable being seen in public. The lips take around three months to settle into their final shape. Expect:

  • The most swelling in the first two to three days, with most visible swelling gone over about three weeks.

  • Stitches removed, or dissolved, within about a week.

  • A soft diet for up to ten days.

  • No strenuous exercise for four to six weeks.

What is a lip dermal graft?

A lip dermal graft (also called a dermal or dermis-fat graft) is a surgical lip augmentation. It uses a strip of your own tissue instead of an injected filler or a synthetic implant. The surgeon takes a small strip of skin from a discreet area, such as the groin, the lower abdomen, behind the ear or an existing scar, and removes the outer surface layer. The remaining dermis is then passed through small incisions into the body of the lip. What is the typical recovery timeline for a lip dermal graft?

Recovery has two parts: the lips and the donor site. The lips swell the most and take the longest to settle, while the donor site usually feels like a small, healing cut. Published guidance on surgical lip enhancement from The Aesthetic Society puts typical downtime at one to two weeks. The stages below are a general guide; your own recovery will depend on your tissue and on whether you have other procedures at the same time.

Days 1 to 3

Swelling, tightness and bruising are at their worst, with mild to moderate discomfort. Cold compresses help. You may need liquids or very soft food, and you should keep facial movement, talking and smiling to a minimum. Sleep with your head raised.

Days 4 to 7

Swelling starts to ease, although your lips will still look larger than the final result. Stitches are usually removed at around four to six days, or they dissolve on their own. Most people can manage light activities, but stick to soft food.

Week 2

Most of the bruising has faded. Many people return to work and normal daily activities at this point, though some swelling remains and the lips may feel firm or slightly uneven. Keep to a soft diet for up to ten days if your surgeon advises it.

Weeks 3 to 6

Visible swelling settles over about three weeks, and the lips gradually soften. Avoid strenuous exercise until four to six weeks after surgery, or until your surgeon clears you.

Three months and beyond

The lips reach their settled shape at around three months. Judge symmetry and size only after this point, because uneven swelling in the early weeks is common and usually temporary.

How long should I stay in Bangkok after a lip dermal graft?

Plan on seven to ten days. The American Society of Plastic Surgeons suggests waiting seven to ten days after facial procedures before flying. That window also lets your surgeon check the lips and the donor site, and remove any stitches, before you leave. Will my lips still be swollen when I fly home?

Usually, yes. At seven to ten days your lips will normally still be larger and firmer than the final result. That is expected and is not a sign that anything has gone wrong. Pack lip balm for the dry cabin air, drink plenty of water and keep taking your medicines on Bangkok time until you land. Then judge your result at three months, not on the flight home.

How do I care for the donor site after a lip dermal graft?

The donor site is closed with stitches and usually heals like a small surgical cut. Keep it clean and dry as instructed, avoid friction from tight waistbands if it is in the groin or abdomen, and attend your stitch check. Once it has healed, scar care such as silicone gel can help the scar fade.

What can slow recovery or cause problems after a lip dermal graft?

Most recoveries are uneventful, but you should know what to watch for:

  • Smoking and vaping reduce blood supply to the graft. Stop well before surgery and do not restart during recovery.

  • Cold sores: If you get them, tell your surgeon. Surgery around the mouth can trigger an outbreak, and preventive medicine may be advised.

  • Infection: Increasing pain after day three, spreading redness, pus or a fever need prompt review.
