Overview
What are mesenchymal stem cells?
Mesenchymal stem cells (MSCs) — also called mesenchymal stromal cells — are multipotent adult cells first identified in bone marrow and later found in many other tissues. They are defined by the International Society for Cell & Gene Therapy (ISCT) by three criteria: they adhere to plastic in culture; they can differentiate into bone, cartilage and fat cells; and they carry a characteristic surface-marker profile (positive for CD73, CD90 and CD105, and negative for markers such as CD34, CD45 and HLA-DR).
Two properties make MSCs a major focus of regenerative medicine. First, they are multipotent — able to become several connective-tissue cell types. Second, and increasingly understood as their primary therapeutic mechanism, they act through paracrine signalling: rather than simply replacing damaged cells, MSCs secrete a rich mixture of growth factors, cytokines and extracellular vesicles — their "secretome" — that can influence surrounding cells, support tissue repair, and help regulate immune and inflammatory activity.

The biology in brief
Decades of peer-reviewed research point to a consistent set of mechanisms behind the interest in MSC therapy. These are the properties most frequently described in the scientific literature.
Multipotency
MSCs can differentiate into several connective-tissue lineages — osteoblasts (bone), chondrocytes (cartilage) and adipocytes (fat) — one of the defining ISCT criteria.
Paracrine secretome
Now regarded as their main mode of action: MSCs secrete growth factors, cytokines and extracellular vesicles that signal to nearby cells and support tissue repair, angiogenesis and reduced fibrosis.
Immunomodulation
MSCs interact with both innate and adaptive immune cells — T cells, B cells, NK cells, macrophages and dendritic cells — and generally help calm an over-activated immune response toward balance.
Homing & low immunogenicity
MSCs can migrate toward sites of injury and inflammation, and their low expression of HLA-DR is why donor (allogeneic) cells can often be used without strong immune rejection.
Where they come from
Sources of mesenchymal stem cells
MSCs can be isolated from several tissues, broadly grouped into adult and birth-associated (neonatal) sources. Each has a different profile; the appropriate source for any individual is a clinical decision made by the treating team.
- Bone marrow (BM-MSCs) — the original and most-studied source in clinical trials.
- Adipose tissue (AT-MSCs) — abundant and accessible, with strong proliferative capacity.
- Umbilical cord / Wharton's jelly — a neonatal source noted for high proliferation and a long lifespan.
- Placenta & amnion — birth-associated tissues expressing more primitive markers.
- Dental pulp & peripheral blood — additional adult sources described in the literature.
- Autologous or allogeneic — cells may be your own, or from a screened donor.

Evidence & safety
What the research does — and doesn't — yet show
We believe patients deserve a straight account. MSC therapy is one of the most actively researched areas in regenerative medicine: by 2023, more than 1,100 MSC clinical trials had been registered worldwide, and safety across early-phase studies has been broadly reassuring. A small number of MSC products (around a dozen) have received regulatory approval in specific countries for specific conditions — most of them in Asia.
At the same time, for most indications the evidence for efficacy is still developing, and results vary between individuals. MSC therapy is not a cure-all, and outcomes cannot be guaranteed. Because of this, we describe MSC therapy honestly as a field of active research rather than a proven treatment for any specific disease.
